About to be discharged, but not quite ready to return home? Alta Vista Regional Hospital offers extended care designed to meet the individual needs of patients who are in a transition phase of illness or recovery. Patients may be able to participate in the program if their medical needs cannot be met at home, but their condition no longer requires a stay in an acute care setting. We can provide continued recovery services that are designed to meet each patient’s individualized needs.
After completion of an acute care three-night qualifying stay, our program can allow patients to rehabilitate in our hospital setting while remaining close to home. This may include care after an acute stroke, after orthopedic type surgeries like a total joint replacement or fixation of a fracture, or after acute medical illness that required hospitalization. Patients can receive around-the-clock, individualized care from our licensed nursing staff. The patient’s specialist will continue to remain involved in their care, with on-site consultations or visits to their office.
If you would feel more comfortable with the safety of a hospital environment, healing can begin here at Alta Vista Regional Hospital. Patients can transfer from any acute care hospital to our swing bed program.
